Van Buren County Hospital to Offer Sports Physicals

Van Buren County Hospital will again be offering FREE Sports Physicals! This year they are offering three convenient days and times for the physical appointments!
Clinics will be held on:
Thursday, July 18th | 9-1
Thursday, July 25th | 11-4
Monday, August 5th | 1-5

Please have all forms completed and signed before your scheduled appointment.
If parents WILL NOT be attending with their student a Parent Consent Form is required! You can find that form here: Parent Consent Form.
