VBCH makes it as easy as possible to transfer between levels of care.
Social services and discharge planning are initiated at the earliest possible time during a patient’s hospitalization to optimize patient and family outcomes.
Patients and family members are provided information on the various levels of care available and the processes involved in transferring from one level of care to another.
Referrals are made when appropriate and patients and family members are assisted as needed in finding alternative facilities for placement.
