Health Information

Van Buren County Hospital’s Health Information Management Department is responsible for maintaining medical records for each patient we treat.

The purpose of Health Information Management is to ensure the accuracy, integrity, and security of the medical record as well as continuously improving access of information while maintaining privacy.

The medical record documents the course of the patient’s care during inpatient or outpatient encounters and serves as a basis for planning and evaluating patient care and providing a means of communication between the physician and other professionals in contributing to the patient’s care.

The contents of the medical records at VBCH exist in paper format prior to September 22, 2012, and in electronic format from September 22, 2012, to the present. Records are in accordance with Iowa State Regulations and Van Buren County Hospital Rules and Regulations. HIPAA, Federal and State laws, determine how personal health information is accessed, used, and retained.

Release of Health Information

You have the right to request copies of your medical record. To request a copy of your record please complete the Release of information form, which can be found below or you can pick one up at the hospital or any of our clinics. Please fill out all sections on this form, sign it and then return.

MyChart

Electronic records are also available to patients via MyChart. Patients who set up a MyChart account are able to see their medical record, such as medical history, current health issues, medication history, and more, all online and up to date.
