The Complex Care Program is looking for a patient-centered Social Worker to join our team. The right candidate for this position will have experience within a healthcare setting, familiarity with insurance qualifications, and have a basic knowledge of community resources available for our patients. This person must be able to work in a fast paced, high volume environment. The person will work collaboratively with multiple team members to seek out, organize, and coordinate services for patients and their families within the Complex Care Network. The person in this position may be required to travel to neighboring counties and will transport Complex Care patients. This person will have the opportunity to help and be a participant in a new initiative at Van Buren County Hospital and Clinics. A Bachelor's Degree in Social Work, Human Services, or related field is required, license preferred.
