Complex Care Nurse

An innovative new program at Van Buren County Hospital and Clinics is expanding! The Complex Care Program is looking for a patient-centered nurse to join their team and work closely with the program’s medical director. The right candidate for this position must be able to work in a fast paced, high volume environment that prioritizes individual patient needs. The selected candidate will work collaboratively with multiple team members to seek out, organize, and coordinate services for patients and families within the Complex Care Network. The person in this position will be required to travel to outlying clinics and other clinics within the Network in other counties. This position will primarily be in the clinic as the Complex Care Medical Director’s nurse, but they will also be performing care coordination duties. This applicant must hold a LPN or RN licensure in the state of Iowa or compact state.
