Job Coach/Evaluator (PRN)
Van Buren County Hospital's Job Opportunities Program is seeking a compassionate and motivated PRN Job Coach/Evaluator to support individuals with disabilities as they explore employment opportunities, develop workplace skills, and achieve greater independence.
This position works directly with clients in community employment settings, providing coaching, assessment, training, and ongoing support to help individuals succeed in competitive employment. Primary Responsibilities
- Provide individualized job coaching and workplace support to assigned clients
- Assist clients in developing job skills, work habits, and employment goals
- Conduct job assessments and evaluate client performance in community work settings
- Work collaboratively with employers, vocational rehabilitation counselors, families, and support teams
- Document client progress, complete required reports, and maintain accurate records
- Identify barriers to employment and assist in developing strategies for success
- Promote client independence while ensuring a safe and supportive work environment
- Maintain confidentiality and comply with HIPAA and program requirements
Qualifications
Required
- High school diploma or equivalent
- Strong interpersonal and communication skills
- Ability to work independently and manage multiple priorities
- Basic computer skills
- Valid driver's license and reliable transportation
Preferred
- Experience working with individuals with disabilities or other human service populations
- Previous coaching, teaching, mentoring, healthcare, rehabilitation, or vocational services experience
- CPR certification and Mandatory Reporter training, or the ability to obtain after hire
