Skilled nursing care provides an alternative for patients who are recovering from illnesses or injuries and still need nursing care, but no longer at the acute care level.
While these patients generally do not need to see their physicians daily, they do require a higher level of care than is available in many nursing homes or similar facilities.
Nurses provide medication and treatment and monitor patients’ conditions. All members of the health care team work closely by participating in weekly multi-disciplinary care conferences to help patients improve their physical, mental and psycho-social well-being.
Any patient receiving this level of care must have a physician’s referral and meet predetermined criteria.
